  1. Ask the patient if they want want the rape kit performed, if they decline, please document in T-system
  2. Please notify the social worker that is on call.

  3. Please notify hospital police as well as NY police department since they will be the ones receiving the rape kit.

  4. Please send all their labs (there is a list in the kit with directions) and a pregnancy test.

  5. Please give medications from the pyxis (Plan B, GC Treatment, HIV prophylaxis) in the ED.  Please give prescriptions for HIV prophylaxis.

  6. On top of the rape kit, you will find the form which is titled New York State Office of Victim Services Medical Provider Forensic Rape Examination Direct Reimbursement Claim Form (second link attached below).  This is a form that must be completed by the medical provider.  You will fill out section 1 and section 3.  This form allows the victim to elect to not bill their insurance and instead bill the NYS office of victim services.  If they would like their insurance billed then you can leave the form blank and notify the registrar.  Once you are done with the form please hand the form to the registrar.  They will submit the form for correct billing.

  7. Please refer patients to Heat clinic (under 25 years old), Star clinic or employee health…based on age of patient and if they are an employee or not.  They can also be referred to their own primary care as well.
